I'm no doctor, but that looks like lymphedema. Do you feel any pain?

I do not get any pain or any discomfort as such.
That's good. Have you seen a doctor about this recently?
Even if this isn't lymphedema, cankles are a sign of an underlying health issue like medication complications of insufficient blood flow.
