I have a very specific desire. I want to make my primary care doctor mad/concerned with the amount of weight I've gained. To date, she has never seriously addressed my weight. At 395 lbs this is odd, of course. Perhaps she is a closeted FA?
How much do you suppose I'll need to gain in the next 3-4 weeks before my next follow up for my blood pressure/other random health issues? Thinking if I weigh in around 420/425 that should do the trick. Would be my highest ever recorded weight!

Yes, fatphobic doctors are a thing. However, in recent years, the medical industry is working on dismantling medical fatphobia as studies show it decreases patient level of care. And so unless your fatness is causing you major health issues, many of them will not comment on it much if at all.
It's also highly unlikely that your doctor has any kinds of feelings for you outside of finding your pleasant or not. (Fun fact! Your doctor will notate on your medical records if your a nice patient to deal with.) You are more than likely one of several fat men she cares for. She also will probably not risk her medical license do to anything with you for kink purposes.
I do find it curious that you want to mess up your health enough to cause your doctor concern. Because that never ends well for the patient. Unless you are aroused by blood tests, pills, and medical expenses on top of the general pain that comes from high blood pressure.
