My wife is in the odd position where her cardio health is so good doctors have raved about her heart, but she's also clinically super-obese.
Which actually started making health check-ups awkward- for the doctors! The last time we went together, the doctor tried to convince her to lose weight by saying "Your belly must be so uncomfortably heavy!" and "Do you have trouble sleeping?" while she shugged her shoulders and said "Eh...not really. I don't usually notice."
We agreed afterwards he probably had good intentions. She might have been the fattest patient he'd seen, and just assumed she wanted weight loss encouragement.
