It depends on one's fat distribution. If you only had an excess of subcutaneous fat, you could be metabolically healthy even at very high BMIs, like class III obesity. If a high percent of your fat was visceral though, you could develop metabolic syndrome before you're even overweight.

What is metabolic syndrome? I’m overweight with a large waist that is mainly visceral fat.

Metabolic syndrome includes high blood pressure, high blood sugar, excess body fat around the waist, and abnormal cholesterol levels. The syndrome increases a person's risk for heart attack and stroke.

It's basically why most adults in the US are on meds. Like 1 in 6 people have it.
