Yeah she did. It was pretty stressful for us. She made the mistake of buying the dress so early in our engagement. It was this affordable, boho style dress she had ordered online. She was super bummed and pissed off she wouldn't be able to wear it.
About a month out, she realized her arms had become too fat for the sleeves, her belly stuck out against the fabric at her waist, and the zipper at the back couldn't be pulled all the way up without a huge heave. It was skin tight and she didn't like the way it looked, so she ran around trying to find a new dress to fit her fatter body.
Buying that far ahead is risky. My wife bought hers maybe five months before and it was definitely snug by our wedding day, but fortunately the cut and material didn't really show that. If she'd bought further ahead I think she would have been in trouble; certainly by our first anniversary she wouldn't even try it on because she knew it would not fit.

A lot of preventing back pain is strengthening your core. I'd suggest looking up 'mcgill big three back exercises'. Hopefully you can still do them, and build strength as you grow.
Also supportive, soft, shoes so you are not jolting your joints and back.
For non back stuff, just from reading not personal experience,
- a long handled shoe horn, and elastic shoe laces
- a heavy duty folding chair (lawn chair type idea) so that you are not worried about having a place to sit at outdoor events

Personally I take perverse pleasure in pre or post eating, especially when it is substantial (like getting a full fast food meal on the way to eat dinner with people). So not eating TOO heavily when out with others doesn't bother me.

A bunch of factors.
- moving more mass takes more energy, so every movement of a heavier person takes more energy than for a thin person, and most of that energy ends up as heat that needs to be shed.
- as mentioned fat is a pretty good insulator, and in particular as you get notably fat it tends to start showing up in places where we have major blood flows near the skin, putting insulation where other people have none (such as fat necks and knees)
- not a hard and fast rule, but a lot of fat people cover up more than their thinner colleagues
- fitness is also a factor in handling heat, and some fat people are also quite unfit.
I'm sure I've missed some other factors, but those should go a long way to explaining bit.

I mostly just go get sweaty. But I do put a lot of thought into clothes because of this. Either changing in and out of 'clothes to swear in' or being careful what I wear.
One lesson came from shoe shopping of all things. (Ironically for sports shoes). Belly had gotten big enough that tying up shoes made it hard to breathe, and the store was a touch warm. I was wearing a light grey that changed to dark grey when damp. When I was finally done (i have hard to fit feet, so tried on quite a few pairs) I had a dark grey line across my front from under moob sweat. So now, even to go shopping, I think about what I'm wearing and will it show sweat?

5) Yo-yo. You gain up to a certain size, then lose some weight and gain it back again ( distinct from yoyo dieting in that the gain is deliberate, and the loss is intended to be temporary).

For starters, there is no substitute for fruits and veggies. He may not love them, but green and orange vegetables in particular do so much for us that supplements can't really fully cover.
If he is getting his veggies he should get most of what he needs. However a serious multivitamin can help make sure he isn't short on some vitamin or mineral (look for the kind with like 20 things in them, or just look for zinc near the end of the ingredients list)
The one thing that I would suggest is that in the winter months (or all year if he avoids the sun) he take a vitamin D supplement. In general people in north America and northern parts of Europe tend to be low on it in the darker half of the year, as we mostly get it from being exposed to strong enough sun, and there is very little in our food. And apparently we use up our stores of it some with eating carbs, so if eating a lot of cheap food he may be more prone to running low.

Ok, so I have more questions than answers for you, for now. There is really two meanings of teasing, one where there is some humiliation involved (and some people like getting that from someone they trust), and the other is more like hints of future pleasures.
For example, let's say his pants are getting tight due to gaining.
-The first type would be to get him to wear those tight pants and take him out to eat a bunch, tell him how it looks like he is going to pop a button, how everyone can see that the piggy is getting too fat for his pants, maybe eventually have him unbutton them and have to walk out that way, showing everyone that he can't even close his pants anymore.
- the second type would be to take him shopping for me pants, and get him to try on not just one size bigger but also two sizes bigger, speculate about how long it will be until he needs that yet larger size, check the slack and talk about how far he a will look once he fills those, and how much you are going to have to get him to eat to get there.
Know which type he wants! Or if in doubt stuck with the second.
And then, just because he wants both of you to gain together doesn't mean that you have to agree. You could decide that you are ok with him gaining but that you don't want to get any bigger. Some of that is just communication and negotiation, but it changes the teasing too. Like if both gaining tease him about needing to reinforce the bed frame soon, if you only want him to gain, tease him about how long it will be before he is heavier than you. (Side note: most people who are into mutual gain have a variety of fat related kinks, so if you don't support all of them that is probably ok so long as you support some of them)

For round numbers, let's call it 100 days (14 weeks plus an extra weekend) So an average of ten dollars a day beyond your usual budget. Not a lot, but fortunately dome calories are cheap! Some of the cheap calories really are not very healthy, but this is a limited time thing so I won't worry about that much.
I'd start with a pint of heavy cream each day, which might cost you $4 or so? Around 900 calories, and many people here have claimed that heavy cream has really helped them gain.
Then you need to get to know the stores in your area. Some sort of low budget store probably has some low priced treats, like a fair sized box of mini donuts for $2.99 or something. Failing all else, pop tarts are usually a calorie bargain. Whatever it is, you are looking for something easy to snack on over the day, totalling up around 1200 calories or more (a box of 8 pop tarts is 1600 calories, btw). Call this another four bucks a day, and want to go through one such thing each day.
This leaves ~$15 a week for extra treats, like ordering a late night pizza or making a fast food run one day or having a lot of soda to drink, or whatever.
It is not as dramatic as you might have been thinking of, but I suspect it would still add up to a pretty noticeable gain.
